What to know before your visit

Please note that we close for lunch from noon to 12:30pm.

If you need to cancel or reschedule your appointment, please let us know at least 24 hours in advance. We do our part to stay on track, although we're aware that your schedule can change because of an emergency at a moment's notice.

Appointment preparations

If your child is one of our new patients, we encourage you to arrive at our office 30 minutes early, so you have time to fill out paperwork. You can even download, print and complete our forms before your appointment if you prefer to save time in advance.

We also ask that you plan ahead as much as possible by finalizing some essential steps, including:

  • Compiling relevant information about your child's medical history
  • Documenting any questions you or your child will want to ask
  • Giving your child a brief description of what to expect
  • Organizing any insurance and financial information, including your health insurance card, your child's Social Security number, their parents' names and your co-payment amount
  • Printing out directions to our office, or calling us for details about where to park and how to find our location

Additionally, please bring these items with you on the day of your visit:

  • List of your child's current medications
  • Recent copies of X-rays and lab results
  • Your insurance card and photo ID
