Our welcoming pediatric otolaryngology specialists look forward to meeting with you and your child. Before your appointment, it's important that you have all the information you need to ensure the best possible experience.
Appointment scheduling
If your original appointment no longer works due to an unexpected emergency, please try and provide at least 24 hours notice.
First-time visitors
We ask you to arrive to your appointment 30 minutes early if your child has yet to see us before. This allows enough time for you to complete all necessary forms. You can also do this ahead of time by downloading, printing and finalizing your paperwork from the comfort of your own home.
What to bring to your visit
In order for your child to be seen, you must provide us with the following:
- Completed forms (if you filled them out in advance)
- Copies of your child's recent X-rays and lab results
- Current medications your child is taking
- Your photo ID and health insurance card
