What to know before your arrival
Prior to visiting one of our clinics, there are some details you should be aware of that can help you and your family have the best experience possible.
If you need to reschedule or cancel your appointment, we require a 24-hour notice. For your child's first visit with us, we ask that you arrive 30 minutes early to complete our paperwork if you haven't already done so in advance.
You can also assist our pediatric oncology and hematology teams by following each of our requests below, including:
- Bring the following to your appointment: completed forms, a list of your child's current medications, recent copies of their lab results and X-rays and your photo ID and health insurance card
- Call for details about where to park and how to find us
- Compile your child's relevant medical history information
- Give your child a description of what they should expect
- Jot down any questions you both might want to ask
- Prepare your child's Social Security number, their parents' names and your co-payment
